ABSTRACT

Objective@#To investigate the influence of pharmaceutical care pathway on cancer-related fatigue (CRF) and negative emotion in patients with advanced lung cancer undergoing chemotherapy.@*Methods@#Patients with advanced lung cancer admitted to the oncology department of Yongkang first people′s hospital from January 2017 to June 2018 for chemotherapy were randomly divided into two groups:61 cases received conventional clinical pathway management (control group), and 64 cases received conventional clinical pathway combined with pharmaceutical pathway management (intervention group). Fatigue, anxiety, depression scores were compared between the two groups before and after 2, 4 cycles chemotherapy.@*Results@#There was no significant difference in baseline condition between the two groups (P>0.05). After 4 cycles of chemotherapy, emotional, physical and cognitive fatigue scores in the intervention group were lower than control group (t=2.975, 1.512, 2.393, P<0.05). With the increase of chemotherapy cycles, scores of fatigue dimensions in control group were increased (F=44.274,21.740,29.261,7.206, P<0.05). In the intervention group, there were statistically significant differences in scores of fatigue dimensions (F=17.083, 15.902, 17.055, 10.543, P<0.01), among which, scores of behavioral and physical fatigue dimensions were increased (P<0.01), while scores of emotional and cognitive fatigue dimensions were decreased (P<0.01). In terms of negative emotions, depression scores in the intervention group were lower than those in control group at 2 cycles of chemotherapy (t=2.578, P<0.05), and at 4 cycles, anxiety and depression scores were lower than control group (t=7.004, 7.454, P<0.05). As the chemotherapy cycle increased, anxiety and depression scores in the control group increased (F=12.333, 14.819, P<0.05), while those in the intervention group decreased (F=4.992, 3.826, P<0.05).@*Conclusion@#The pathway of clinical pharmaceutical care can significantly improve the CRF and negative emotions of patients with advanced lung cancer undergoing chemotherapy.

ABSTRACT

Objective To investigate the expression and clinical significance of plasma miR-124 in acute ischemic stroke (AIS). Methods Forty patients with AIS were collected and 40 volunteers without history of AIS were set as control. Infarction volume was detected by MRI; plasma miR-124 expression level was measured by RTPCR technique and neural function was evaluated by NIHSS scores. Results Compared with that in the control group, plasma miR-124 level in AIS group was significantly reduced (P < 0.05). Plasma miR-124 level in AIS patients with infarction volume greater than 3 cm3 was significantly lower than that of AIS patients with infarction volume less than 3 cm3 (P < 0.05). Correlation analysis showed a negative correlation between miR-124 and infarction volume (r =-0.473, P < 0.05). Plasma miR-124 level in AIS with NIHSS score higher than 5 was significantly lower than that of AIS patients with NIHSS lower than 5 (P< 0.05). NIHSS score negatively correlated with the miR-124 level of AIS patients (r =-0.567, P < 0.05). Conclusion The plasma miR-124 is significantly reduced in patients with AIS, and negatively correlated with the cerebral infarction volume and NIHSS score.

ABSTRACT

Objective To investigate the influence of the microscale attractive interaction on the elastic properties of DNA film in multivalent ion solutions. Methods Kornyshev's electrostatic zipper model was employed to describe the interaction energy between the DNA strands. The thought experiment method and macroscopic continuum bar model were combined to predict the stress-strain relationship, prestress, and elastic modulus of the DNA biofilm.Results Given the packing conditions, the DNA film exhibited a tensile prestress and negative elastic modulus. The prestress of the DNA biofilm ranged from -1.52 MPa to 1.17 MPa, and its elastic modulus ranged from -4.2 MPa to 64 MPa. Conclusions In contrast with monovalent solutions, the microscopic attractive interactions in multivalent solutions caused the elastic properties of the DNA film to exhibit a non-monotonous relationship with the variation in the packing density and salt concentration. The tensile elastic properties were significantly different from the compressive ones, and the tensile/compressive prestress as well as the positive/negative elastic modulus transformed each other. These results can contribute to understanding the mechanism of viral replication and provide references for gene detection and gene therapy.

ABSTRACT

Objective To explore the value of whole-lesion method measuring hepatic abscess lesions in ADC map.Methods 13 cases with hepatic abscess were measured by using three methods,drawing the outline of the lesion by manual operation,and placing the round ROI in the central area and the peripheral area in ADC map,respectively.The differences of the ADC values measured by the three methods were compared.The Friedman test was used for the mean of ADC values.The value differences of the groups were compared by the Bonferroni test.Results Among the 17 lesions,6 lesions located in the anterior segment of right lobe of liver,7 lesions in the posterior segment of right lobe of liver,1 lesion in the caudate lobe,1 lesion in the junction zone between the anterior segment of right lobe and the medial segment of left lobe,1 lesion in the medial segment of left lobe and 1 lesion in the lateral segment of left lobe of liver.The mean values from the three methods(×10-3 mm2/s) and the 95% confidence intervalwere:1.27±0.33,1.23± 0.84,1.29±0.44 and (1.21,1.34),(1.06,1.40),(1.20,1.38),respectively.The results of the Friedman test were x2=2.176,P=0.337,and there was no statistical significance between the differences.There were also no statistical significance by the Bonferroni test (P values >0.05/3=0.017).The 95 % confidence interval of the whole-lesion method was the narrowest,and the accuracy was the highest.The 95% confidence interval of the central method was the widest,and the accuracy was the lowest.Conclusion The whole-lesion measurement may be used as an analytical method for hepatic abscess in ADC map.

Objective To systemic evaluate postoperative pulmonary function between segmentectomy and lobectomy in patients with early stage non-small cell lung cancer.Methods Computer searched was performed up to September 2016 at the Cochrane Library,PubMed,EMBASE,CBM,CNKI,VIP and Wanfang database,collected postoperative pulmonary function between segmentectomy and lobectomy in Patients with early stage Non-Small Cell Lung Cancer.Study selection,data collection and critical assessment of the included studies were performed according to the recommendations of the Cochrane Collaboration.Results Eight studies included 308 segmentectomy and 386 lobectomy were identified for inclusion.FVC% (MD =9.91,95% CI:3.53-16.30,P =0.002),postoperation / preoperation FVC% (MD =6.74,95% CI:5.52-7.79,P <0.05),FEV1% (MD =13.25,95% CI:10.25-16.26,P < 0.05),postoperation / preoperation FEV1% (MD =7.18,95 % CI:5.88-8.48,P < 0.05) showed significant difference.Conclusion Pulmonary function index was superior to lobectomy after segmentectomy in patients with early stage lung cancer,and segmentectomy resection was more beneficial to preserve lung function.

ABSTRACT

Objective To evaluate the ability of ultra-high-b-value diffusion weighted imaging (DWI) in diagnosis of the prostate cancer (PCa) at 1.5T MR.Methods 12 patients with PCa and 17 patients with benign prostate hyperplasia (BPH) proved by histopathology underwent MRI and DWI examinations (b=400, 800,1 400 s/mm2).The signal intensity and visual degree of region of interest (ROI) in all DWI with different b values were respectively measured.The signal intensity and visual degree of ROI between high-b-value DWI and ultra-high-b-value DWI were compared,respectively.Results The difference of signal intensity of PCa between high-b-value DWI and ultra-high-b-value DWI was statistically significant (χ2=220.957,P=0.000<0.05).The signal intensity of PCa was getting brighter in DWI with higher b value.The difference of visual degree between these two groups was also statistically significant (χ2=11.378,P=0.003<0.05).The difference among PCa, BPH and normal prostate peripheral zone was statistically significant in ultra-high-b-value DWI (χ2=25.913, P=0.000<0.05).The brightness of PCa in ultra-high-b-value DWI mainly was bright or grey-bright (71.4%), while the brightness of BPH and normal prostate peripheral zone were mainly dark or grey-dark (BPH 63.0%, normal prostate peripheral zone 73.3%).The difference of visual degree among PCa, BPH and normal prostate peripheral zone was statistically significant difference as well (Z=-6.908,-6.110,P=0.000<0.017).The diagnostic efficiency of the signal intensity and visual degree were highest with b=1 400 s/mm2.Conclusion DWI with ultra-high-b-value can improve the display rate of the PCa at 1.5T MR, making easier the detection and diagnosis of PCa.

ABSTRACT

<p><b>OBJECTIVE</b>To describe clinical and genetic feature in a Chinese family with familial idiopathic basal ganglia calcification 3 (IBGC-3) caused by a novel mutation in the SLC20A2 gene.</p><p><b>METHODS</b>Clinical data was collected from a family with familial IBGC-3. All of the family members underwent cerebral CT. Potential mutation of the SLC20A2 gene were screened in the proband, 5 symptomatic patients, 5 asymptomatic family members, and 100 healthy Chinese controls. Exon 8 of the SLC20A2 gene was cloned into plasmid and sequenced.</p><p><b>RESULTS</b>There were 6 symptomatic patients (3 males and 3 females) in an autosomal dominant pedigree. The patients manifested as juvenile-onset paroxysmal kinesigenic dyskinesia, in addition to pyramidal signs in proband. 5 patients alive had calcification in bilateral basal ganglia and subcortical areas. One asymptomatic member also had calcification in the brain; and 2 cases of asymptomatic young members had bilateral globus pallidus calcification. A novel c.1086delC mutation in SLC20A2 gene has been identified in proband and 7 family members with intracranial calcification. The deletion mutation was not found in 2 family members without intracranial calcification and healthy controls members. There is no clear relationship between clinical symptoms and the severity of calcification in cerebral CT.</p><p><b>CONCLUSION</b>Familial idiopathic basal ganglia calcification caused by the SLC20A2 gene mutation can manifest as juvenile onset paroxysmal kinesigenic dyskinesia. Further study should be done to validate the unrelated relationships between the severity of calcification in IBGC 3 cranial CT and clinical symptoms.</p>

ABSTRACT

Objective To report the clinical features and inwardly rectifying potassium channel 18 (KCNJ18) gene mutation in a group of patients with thyrotoxic periodic paralysis (TTP).Methods Fiftyseven TTP cases (55 male and 2 female) were collected in our clinic from July 2002 to October 2011.The KCNJ18 gene was directly sequenced in 57 TTP patients and 50 health Chinese controls through the nested PCR.According to the results of gene screening,the clinical features of KCNJ18 patients and non-KCNJ18 patients were retrospectively summarized and analyzed.Results In 4 male patients with TPP,we found 3 novel heterogeneous mutations (p.Q126X,p.K360T,p.E388K) and 1 reported mutation (p.A200P) in the KCNJ18 gene.The age of onset was 19-25 years old,and the duration ranged from 2 to 8 hours.The 4 patients all presented severe muscle weakness.The attacks of muscle weakness preceded overt symptoms of hyperthyroidism in the 4 patients. Three patients showed recurrent weakness during the 13-28 months follow-up,while the episodic weakness never appeared when patients got euthyroid. Conclusions The mutations in the KCNJ18 gene are responsible for a part of Chinese patients with TPP.The patients with KCNJ18 mutations have a shorter disease course,severer manifestation,and higher prevalence of recurrence as compared with those TPP patients without KCNJ18 mutations.

ABSTRACT

<p><b>BACKGROUND</b>To investigate the expression features of the heterogeneous nuclear ribonucleoprotein A2/B1 (hnRNP A2/B1) in non-small cell lung cancer and its clinical significance.</p><p><b>METHODS</b>hnRNP A2/B1 expression of cancer tissues, paracancerous lung tissues, resected bronchial stump epithelium tissues was detected in 58 non-small cell lung cancer patients and pulmonary tissues in 30 patients with benigh pulmonary lesions as control by immunohistochemistry methods.</p><p><b>RESULTS</b>The positive expressive rate of hnRNP A2/B1 in lung cancer tissue (63.79%) was significantly higher than those in paracancerous lung tissue (43.10%) and benign pulmonary lesion tissues (20.00%) (P=0.000), and the positive rate in paracancerous lung tissue was also significantly higher than that in benign pulmonary lesion tissues (P < 0.05). The positive rate of hnRNP A2/B1 in the hyperplastic and dysplastic epithelium (40.00%) of resected bronchial stump was remarkably higher than that in normal bronchial epithelium (15.15%) in lung cancer patients (P=0.032). The positive rate of hnRNP A2/B1 in poor differentiated lung cancer (78.13%) was remarkably higher than that in moderate and well differentiated lung cancer (46.15%) (P < 0.05). The positive rate of hnRNP A2/B1 in lung cancer with lymph node metastasis (75.00%) was significantly higher than that without lymph node metastasis (50.00%) (P < 0.05). The positive rate of hnRNP A2/B1 in stage III+IV disease (78.57%) was significantly higher than that in stage I+II disease (50.00%) (P < 0.05). The positive rate of hnRNP A2/B1 in T3+T4 cancer (77.42%) was significantly higher than that in T1+T2 cancer (48.15%) (P < 0.05). The positive rate of hnRNP A2/B1 expression in lung cancer tissues was not related to histological classification of the cancer (P > 0.05).</p><p><b>CONCLUSIONS</b>The overexpression of hnRNP A2/B1 in cancer tissues may play an important role in the oncogenesis, development and metastasis of lung cancer. Detection of hnRNP A2/B1 expression may be helpful to diagnose lung cancer and to predict prognosis of the patients with lung cancer.</p>
